Declaration

Please make sure you have read and understood the following declaration before submitting information to Directions. As part of the 'Submission Process' you will be asked to sign a copy of this declaration, which you will recieve through the post, following your initial submission. Please note that without a signed declaration, information can not be added to the database. This process in different for online applications, you will be required to tick a box to agree that the informaiton you have supplied is true and correct.

I/We understand that the personal data which I/we have supplied on this form will be held and processed, by Durham County Council, for the purpose of administering the Directions Information Service. The Directions service provides details of the various social, health, family and disability services that are available to members of the public. Information is offered via a telephone helpline, and is also made available on the Internet, via the Durham County Council website.

Note: Confidential Information will not appear in any of the above formats. Durham County Council is the Data Controller for the purposes of the Data Protection Act.

If you agree to the information you have supplied being processed as described, then please sign the declaration when you recieve it and return it to Directions via post.
